It is possible that readers of this foreword would benefit from pausing for a moment to reflect on the origins of the examination, both intellectually and in practice, as well as the process by which it developed from a collection of disparate insights and models from a variety of fields into a subject that is sufficiently coherent to warrant an entire encyclopedia being devoted to it.
